Respiratory Health Findings and Lung Function Studies - risk of Lung Cancer/Lung Disease

Vaping health effects study

Respiratory health remains one of the most scrutinised aspects of vaping research, given the direct inhalation involved in the practice. Recent studies have shown that while vaping is not without risks, the respiratory impact appears significantly different from that of traditional cigarette smoking.

Research conducted on lung function has revealed that former smokers who switched to vaping often experience improvements in respiratory symptoms within months of making the transition (1). Studies measuring forced expiratory volume and other lung capacity metrics have documented measurable improvements in many participants who completely replaced cigarettes with vaping products.

However, researchers have also identified specific concerns related to certain vaping ingredients. The heating of vegetable glycerine and propylene glycol, while generally recognised as safe for ingestion, produces different compounds when vaporised and inhaled. This process generates E Cigarette aerosol, which contains harmful chemicals such as formaldehyde and acrolein. These substances are present at significant levels in some studies and are known to cause irreversible damage, including small airway fibrosis and constrictive bronchiolitis, however the levels present in E-Cigarettes can be anywhere from 10 to 100 times lower than tobacco smoking. Research indicates that E Cigarette exposure, especially acute exposure, can lead to oxidative stress, DNA damage, and cellular apoptosis, all of which may increase cancer risk. The National Academies of Sciences, Engineering, and Medicine concluded that e-cigarettes emit potentially toxic substances that pose health risks. Vaping has also been linked to severe lung diseases, including lipoid pneumonia and eosinophilic pneumonia (3). Additionally, current vapers have a 39% higher odds of self-reported asthma compared to non-users, and vaping is associated with an increased risk of asthma exacerbation and impaired lung function.

EVALI (e-cigarette or vaping product use-associated lung injury) has been linked to contaminated vaping products, particularly those containing additives like vitamin E acetate. It is important to note that E-Cigarettes made in the UK are prohibited from containing chemicals known to be harmful when inhaled, including vitamin E acetate and diacetyl. Instances of EVALI are almost exclusively existent in the United States (4), where vitamin E acetate has been used to add illicit compounds like THC - the psychoactive component in cannabis - into vape juice. 

When comparing study populations, findings show that 'non smoker current vapers' often exhibit different or lower risks for certain biomarkers and diseases compared to 'non vaper current smokers', who continue to smoke conventional cigarettes while using e-cigarettes, highlighting significant differences in risk profiles.

The absence of combustion in vaping eliminates many of the toxic compounds found in cigarette smoke, including tar and carbon monoxide. This fundamental difference has led many researchers to conclude that while vaping may not be completely risk-free, it represents a substantial reduction in respiratory harm compared to smoking traditional cigarettes - a stance adopted by Public Health England and the NHS. Cardiovascular Impact and Heart Health Research

Cardiovascular research represents another critical area of vaping health studies. There is substantial interest in the cardiovascular effects of vaping, and recent meta-analyses and data analyses have provided new insights into these risks. The relationship between nicotine and heart health has been extensively studied in the context of traditional smoking, but vaping presents a different delivery mechanism that researchers are working to understand fully.

Studies examining heart rate, blood pressure, and arterial function in vapers have produced mixed results compared to smoking. Notably, vaping impairs endothelial function and increases arterial stiffness and blood pressure. Long-term vaping can also impair blood vessel function and raise blood pressure and heart rates. While nicotine itself can cause temporary increases in heart rate and blood pressure, the absence of carbon monoxide and other combustion-related toxins in E-Cigarette vapour appears to reduce some of the cardiovascular risks associated with smoking.

A 2025 meta analysis found that vapers have 53% higher odds of myocardial infarction (heart attack) compared to non-users, indicating a higher risk for heart attacks among e-cigarette users, where smoking is associated with 200% to 400% risk compared with non-smokers.

Additionally, a 2024 NIH data analysis showed that ever-users of e-cigarettes have a 19% increased risk of heart failure, but the median age of the participants in the study was 52, so it is not clear about the associations of previous cigarette smoking on the impact of this figure. (6)

Long-term cardiovascular studies are still ongoing, as researchers require extended observation periods to draw definitive conclusions about heart disease risk. However, preliminary findings suggest that individuals who completely switch from smoking to vaping may experience health improvement in several cardiovascular markers, including improved circulation and reduced inflammation.

It’s important to note that the cardiovascular effects of vaping appear to vary significantly based on the nicotine concentration used and individual health factors. Research continues to explore these variables to provide more personalised health guidance for different user groups.

Comparative Risk Assessment: E Cigarettes vs. Traditional Smoking

Nicotine is the main addictive substance in tobacco products, including e-cigarettes, and can lead to a cycle of addiction characterised by cravings and withdrawal symptoms. Signs of nicotine addiction include strong cravings, inability to stop using nicotine, and developing a tolerance, as well as irritability which can negatively impact personal relationships and performance in daily activities. Withdrawal symptoms may include anxiety, stress, and depression, often leading individuals to continue using tobacco products to alleviate these feelings. Notably, nicotine exposure before age 25 can permanently harm parts of the brain responsible for attention, learning, and mood control. (7)

Additionally, health risks associated with vaping and tobacco products may vary across different populations, such as age, sex, gender, ethnicity, and socioeconomic status, influencing health outcomes.

Studies have revealed significant variations in safety profiles between different types of vaping devices and e-liquids, emphasising the need for consistent regulatory oversight and quality control.
